Eye Care Associates Patient Rewards Program

The Eye Care Associates Rewards Program is your chance to help your friends, neighbors and co-workers save on their eye health beginning with their very first visit.

You earn credit - called Rewards Credit - for referring new patients to our practice. You can use your Rewards Credit to purchase your own prescription eyeglasses, contact lenses or premium sunglasses.

The Math Is Simple

Each qualified new patient you refer equals $20 in Rewards Credit for you to use toward the purchase of your next pair of glasses, annual supply of contact lenses or sunglasses.

Plus - You receive $10 Rewards Credit for every $100 you spend for materials at any of our offices. Just show us your Referral Rewards Member Card every time you come to one of our offices.

And if that isn't enough, you receive a jump start of $70 Rewards Credit just for enrolling in our Referral Rewards Program!*

How Our Rewards Program Works

How To Get Started

You start by filling out the enrollment form below.

In 2 to 4 weeks, we will send you ten (10) $50.00 Instant Discount Cards to distribute to anyone outside your immediate family you think might need our services.  Each Discount Card is worth $50.00 off the purchase of any complete pair of prescription eyewear.

When your friend, neighbor or co-worker uses the $50 Instant Discount Card you gave them, they receive an instant $50.00 discount towards the purchase of any complete pair of prescription eyewear during their first visit to any of our offices (if he or she is not already an Eye Care Associates patient).

Each time a new patient uses one of the cards they received from you, you receive a $20 Rewards Credit.  That's $20 for every new patient you refer!

Please give only one $50.00 Instant Discount Card per new patient referral because discount cards may not be combined with each other (example: 2 cards do not equal a  $100 discount).  Patients referred prior to joining the Patient Rewards Program cannot be credited to you.  For more details, please review the Program Rules & Important Disclaimers below.

We'll Notify You Every Time You Have A Referral

We'll send you an email every time someone uses one of the $50 Instant Discount Cards you gave to a new Eye Care Associates patient. The email will show you the balance of Rewards Credit you now have to use for your next purchase of prescription eyewear, contact lenses or sunglasses.

When You Run Out of Cards

When you run out of the first ten we send you, send us an email (to the email address included with your first shipment) and we'll send you more!

Using Your Rewards Credits

Using the Rewards Credit you have earned by referring friends, neighbors or co-workers is easy.  Simply show us your Rewards Member Card during your next visit and you receive discounts up to the amount of Rewards Credit in your account for any eyewear materials you need or want.
Remember, each qualified new patient you refer to Eye Care Associates is another $20 credit for you to use toward the purchase of glasses, contacts or sunglasses at any of our offices.  For example, refer 10 new patients and you earn $200 for your next pair of glasses or annual supply of contacts or new sunglasses - it's that simple.

*Program Rules & Important Disclaimers

  • You must be a current patient and have a recent Invoice Number to enroll. Have your most recent patient invoice handy when you enroll. Look at the top-right corner of the page for the invoice number. The invoice number must match the "Responsible Party" contact information and must correspond to the information you provide in the enrollment form.
  • In order to keep program costs to a minimum, you must provide a valid email to enroll and all required data fields must be completed so that we may accurately credit your referrals to you. Your privacy is critically important - we do not share this information outside our organization.
  • Only one Referral Rewards Member per household is allowed.
  • Your Reward Credits may not be used to satisfy previous, past due or open balances and may only be used for future materials purchases.  Rewards Credits may NOT be used for professional fees.
  • You will only receive Rewards Credit for new patients who present your $50 Instant Discount Cards. Your $50 Instant Discount are assigned to your personal account. You will only receive Rewards Credit if a new patient presents the Instant Discount Card you provided them and the bar code on the reverse side is scanned.
  • New patients are defined as never having been patients of Eye Care Associates. New patients may not come from the same physical address and may not have the same responsible party as the enrolled member.
  • Referrals you made prior to enrolling in the Referral Rewards program are NOT eligible for Rewards Credit.
  • Rewards Credit may only be used for materials (eyeglasses, contact lens materials and sunglasses) provided by Eye Care Associates. Rewards Credit is intended to be used by the enrolled member and member must present Member Card to redeem Rewards Credit.
  • Rewards Credit may not be exchanged or redeemed for cash equivalent. No reimbursements will be issued for unused Rewards Credit. Rewards Credit may only be transferred to immediate family members living in the same physical address as the enrolled member. Rewards Credit  may not be combined with other member Rewards Credit.
  • Your Rewards Credit expires 12 months from your enrollment date.  If you enrolled in our Referral Rewards program prior to 12/31/2010, your rewards credit balance expires on 12/31/2011, and then, will expire annually thereafter.
  • Eye Care Associates reserves the right to discontinue or modify this program at any time without prior notification. Any disputes regarding discounts offered will be resolved by the management of Eye Care Associates at its discretion.
